RELATING TO CORONAVIRUS/COVID-19

For CDAIDE Staff, Volunteers, and Care Recipients

REVIEW AND ACCEPT

The novel coronavirus, COVID-19, a declared worldwide pandemic by the World Health Organization, is extremely contagious.  COVID-19 spreads primarily from person-to-person contact, including individuals without disease symptoms. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and face masking and have, in many locations, prohibited or limited the congregation of groups of people.

At CDAIDE, the health and well-being of hospitality workers, volunteers and staff remains our top priority. In order to minimize the risk of COVID-19 entering our environment and spreading amongst our community, CDAIDE recommends the following:

​

  1. Wear a face covering that thoroughly covers nose and mouth (unless physically distanced by 6 feet outdoors).

  2. Regularly and thoroughly clean your hands with an alcohol-based hand sanitizer or wash them with soap and water. 

  3. Maintain 6 feet of distance between yourself and others.  

  4. Avoid going to crowded places.  

  5. Avoid touching your eyes, nose and mouth. 

  6. Practice good respiratory hygiene, including covering your mouth and nose with your bent elbow or tissue when you cough or sneeze. Then dispose of the used tissue immediately and wash your hands. 

  7. Stay home and self-isolate even with minor symptoms such as cough, headache, or mild fever, until you recover. 

  8. If you have a fever, cough and difficulty breathing, seek medical attention; call by telephone in advance if possible and follow the directions of your local health authority. 

  9. Keep up to date on the latest information from trusted sources, such as your local and national health authorities.

CDAIDE staff, volunteers and care recipients who decide to meet in person must agree to and abide by the nine steps outlined above. We also recommend meeting in a public place outdoors, where virus transmission is lower than in indoor spaces, when weather permits. CDAIDE cannot guarantee that you will not become infected with COVID-19. 

 

AGREEMENT

I understand that I have the option to carry out any and all activities with CDAIDE (whether as a volunteer, board member, staff member, or care recipient) on a remote basis that does not require person-to-person contact, during the period that COVID-19 remains a threat in my community.  If I am meeting in person for CDAIDE business, it is my choice and preference to meet in person rather than remotely (phone or virtual).  I understand I may request a remote meeting or conversation instead of an in-person meeting at any time. 


By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily agree to the participation terms described above and assume the risk that I may be exposed to or infected by COVID-19 by attending face-to-face CDAIDE meetings. I understand that the risk of becoming exposed to or infected by COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, CDAIDE staff, volunteers and care recipients. 


I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself. I hereby release, covenant not to sue, discharge, and hold harmless CDAIDE, its employees, agents, representatives, and any facility used of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of CDAIDE, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any CDAIDE activity.
